This form is designed to provide students, employees, faculty, vendors, visitors, or others with an on-line method to report specific information related to an alleged incident(s) of sexual and/or Gender-Based misconduct.

You are not required to complete the entire form in order for the complaint to be submitted. The Office of Title IX and Clery Compliance will use the information provided to begin the response process, which may include contacting the complainant, respondent, and/or any potential witnesses. However, if the report does not contain specific information, the university's response may be limited.

UNCW reviews all complaints submitted via this form, and determines the most appropriate response.

Additional Documentation and Information

Note: A copy of this report will be sent to the email listed under "Reporter Information" for your records. This report will be sent to the Director of Title IX, Deputy Title IX Coordinator, Director of CARE, Title IX Administrative Specialist, and a designee of General Counsel. Faculty/Staff Involved Reports will also be sent to a designee of Human Resources. Please add any additional documents that support your complaint. 5GB maximum total size.
