Return to School Preference Planning Survey
Please indicate your intentions for the return to school in January.  Of course if there are changes at the state and district level, there will be adjustments to the current plan.  Thank you!!

I understand that students who choose to return will be assigned to either an "A" cohort or a "B" cohort.  "A" cohort students will attend classes in person on Monday and Tuesday, and receive virtual instruction Wednesday through Friday.  "B" cohort students will receive virtual instruction Monday through Wednesday, and will attend in person at school on Thursday and Friday. *
I understand that students will be checked in each morning with temperature checks and COVID questions, and will remain socially distanced throughout the day.   Students and staff will be required to wear masks throughout the duration of the school day (with the exception of breakfast and lunch). *
I understand that if my student has been around anyone who has COVID recently, is showing symptoms, has a fever, has been tested and is awaiting results, or if someone in the house has been testing and is awaiting results that they may not come to school. *
I understand that students will eat breakfast and lunch in their classrooms, and will not be able to congregate before or after school. *
I understand that families choosing to remain all virtual will receive virtual instruction from their regular teachers; there will be no changes to what they are doing now. *
I understand that This informal survey is for planning purposes ONLY, and there will be additional steps to take if you choose to go all virtual. *
