To order more PCR COVID testing supplies from the State Lab, use the Public Health Test
Supply order form: https://tinyurl.com/46uytf5a. We recommend maintaining a one month supply of swabs, transport media, and biohazard bags for collecting and transporting specimens. When ordering supplies, allow two (2) weeks for delivery.
If you are a residential care facility, work with your assigned lab to order supplies.
If your facility would like to request personal protective equipment (PPE), such as masks, gowns, and gloves from the State, contact your local public health agency or office of emergency management. You can also purchase PPE. If you have any questions, email cdphe_labcoordinators@state.co.us.
