Please provide supporting documentation for the violation as a PDF file. This might include items such as the student’s paper along with the Turnitin report, copies of the web pages utilized, a copy of two similar assignments, comparison of the student’s work with a solutions manual, etc. If you have more than one PDF document please submit them individually*

Your report and accompanying files will be automatically sent to the Associate Academic VP (AAVP) for review. Once reviewed, the AAVP will contact you if any clarification is needed. A copy of the information you submitted along with a cover letter from the AAVP will be emailed to the student.
